Integrated Substance Use Disorder in Primary Care Fellowship

The primary goals of this program are to:
1) Develop APRN fellows into independent practitioners and scholars that can have high impact endeavors in a variety of primary care and teaching environments;
2) Advance fellows into autonomous clinician educators who possess in-depth therapeutic knowledge in caring for patients with substance use disorders;
3) Position fellows to be superior candidates for their desired career goals especially in primary/care substance use disorder.
Required Qualifications
1) Applicant must have received a Doctorate of Nursing Practice (DNP) degree in the past five (5) years
2) Board-eligible (or currently board-certified) as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
3) Meet all requirements for employment by the University of Illinois at Chicago, which includes a background check
4) Eligible for APRN licensure in Illinois (or hold active APRN licensure in Illinois)
Candidates must also present evidence of experience or interest in Primary Care and Substance Use Disorder leadership and practice.

NOTE: One of your recommendation letter writers must be your clinical supervisor or academic mentor; indicate as such in the provided "Relationship to candidate" field.
3) An application letter that identifies the applicant's areas of interest in interprofessional team-based care, SUDs, and primary prevention; and outlines career objectives to be achieved through the traineeship, and a proposed plan for implementation.
